My favourite perk is Jury Rigging it's absolutely amazing! With it i can reapair all my things with similar objects. It 'a huge innovation compared to fallout 3

Bloody Mess! With this perk, a minigun can literally become a grinder and shotgun will be more like a grenade launcher except there's no splash damage to friendly npcs.

Mysterious Stranger, it is just so badass for him to whaltz up and shoot a behemoth to death, although he is less than useful when he executes a prisoner you're trying to rough up in VATS.

I like that one perk that lets you get the blank magazines out of all those useless burned books in the world.. it actually makes them useful for collecting but I never can remember its name til I see it in the list.

For sheer versatility? Jury-rigging is far too useful to not take.
For general use, any of the shotgun-centric perks, they're the clear winner in terms of weapons, unless you're melee and go for Slayer.
For fun? Terrifying Presence.

Like a few said already, jury-rigging.
There's just something magical about fixing the ballistic fist, a glove that shoots shotgun shells, with simple hand-wraps. It makes you the ultimate repair man!
But for fun, sandman, because it plays the xylophone every-time right before you murder a fella.

This one perk called something like "Unbelievably Lucky" because that gives you a small chance of finding unique weapons and clothes in like metalboxes etc.

Jury-Rigging because repairing is extremely annoying in vanilla NV, and jury-rigging makes life so much easier. I also like Nerves of Steel because constantly staying in vats is ridiculously overpowered.

Lonesome Road: 10% damage boost and 10% VATS hit increase chance
This perk makes the Courier a lot more powerful in every way.
